The following contract ensures any claims liability from a defaulting subsidiary will become
the responsibility of the parent organization.
|

Before the text of line 3 of the main body of the form, enter the name of
the subsidiary of the parent employer.

After the text of line 3 of the main body of the form, enter the name of the
parent employer as it appears in the employer’s articles of incorporation.
-
Using normal notary public practices, have the bottom of the form notarized.
