The video library maintains an extensive collection of audiovisual materials (videotapes, DVDs, CD-ROMs) to
supplement your organization’s safety and health training program. All materials are copyrighted.
Video loan request form
The video library requires a signed borrower’s agreement to be on file for each video library borrower. If you are a
first-time borrower, please take a moment to read, sign and send the library the borrower’s agreement on the
video loan request form. You only need to sign
the borrower’s agreement once. You can submit all subsequent orders online, by fax, mail or phone.
Ordering videos
The loan period for videos is 12 days. This includes shipment to and from. The library ships approximately five days
prior to your requested show date. If you need a longer loan period, please call the library. We will do our best to
accommodate, but previously scheduled requests for the same title may limit the library in meeting your request.
For greatest success, the library suggests you place your order at least 30 days in advance of the date you would
like to show the material.
You may place orders up to six months in advance. You may borrow up to four videotapes at one time.
The library will not honor new requests or send scheduled requests if your organization has:
- Material overdue;
- Unpaid charges for lost or damaged material;
- Four videotapes checked out.
It will not ship to:
- A post office box;
- Out-of-state addresses;
- Home addresses.
Many of the videos have accompanying guides, quizzes or CD-ROMs that can enhance your training. The video producers
provide these materials, which vary in length and format. You must return these copyrighted materials with the videos.
Please tell the library when you order whether you want the accompanying guide.
Borrower’s responsibilities
BWC loans videotapes, DVDs and related materials to Ohio employers who have a workers’ compensation account. There is
no direct charge for borrowing materials. However, employers are responsible for:
- Cost of return postage by insured, traceable means;
- Replacement cost of video- tapes, DVDs, and related material lost or damaged while on loan.
For your protection, we ask you return material to the library by insured, traceable means. The extra effort may
cost a few dollars, but may save you hundreds of dollars in the event material is lost or misplaced during delivery.
We have selected each videotape or DVD in its library for the quality of its content. Due to the highly technical
content and limited production runs, we recommend you insure each video for a minimum of $500.
Borrowers must return the videotape(s), DVD(s), or related material to DSH in a box or envelope. Do not attach
labels or tape to the video cases. If you have more than one piece of material, you can ship them together in one box
or envelope.
The video library provides service to several thousand Ohio employers each year. To meet the needs of as many
borrowers as possible, it is critical that you return materials to the library no later than the due date. You will
find the due date noted on your confirmation (e-mail or letter) and on the label on the outside of the video case.
The library will suspend borrowing privileges for a minimum of one month for customers who return materials past
their due date. Failure to return materials by their due date denies other customers use of their expected order.
