The purpose of PERRP is to ensure that public employees in Ohio have a safe and healthful work environment. The public
employer must furnish to each public employee a workplace free from recognized
hazards that are causing or are likely to cause death or serious physical harm.
PERRP services are free and include:
- Safety consultations;
- Safety training;
- Site-specific evaluations;
- Presentations;
- Written program review;
- Industrial hygiene surveys;
Each of these activities is designed to raise the hazard recognition ability of
employers and employees.

Why train employees?
Within the OSHA standards, there are specific requirements for training so that
jobs can be performed in a safe manner.
Why conduct an industrial hygiene survey?
This survey provides an analytical picture of the working environment and in
particular chemical over exposures that employees may experience in the workplace.
Why request a safety consultation?
The development of a detailed facility survey that identifies hazards, location,
standard cited, recommendations and corrective actions are provided to each customer
following a consultative inspection to ensure a safe working environment.
PERRP forms
The PERRP recordkeeping package is designed to help employers complete the Log of Work-Related
Injuries and Illnesses (300P), the Summary of Work-Related Injuries and Illnesses (300AP)
and the Injury and Illness Incident Report (301P). Microsoft Excel is required to download

PERRP Summary of Work-Related Injuries and Illnesses (300AP) – Due Feb. 1 for previous calendar year
All Ohio public employers must submit a Summary of Work-Related Injuries and
Illnesses (300AP) to PERRP. The 300AP form can be

PERRP Sharps Injury Form Needlestick Report (SH-12)
Public employers must complete and submit a PERRP Sharps Injury Form Needlestick
Report (SH-12) for every needlestick or sharps injury that occurs. The SH-12 form can be
