| Introduction |

If you suspect fraud, submit a Fraud Allegation Form online. With this form, you
can provide an effective description of the subjects suspected fraudulent activity.
Special agents from BWCs special investigations department will carefully collect and
analyze the facts in order to determine whether or not fraud was committed.
Importantly, to report workers compensation fraud to BWC, you need only to suspect that fraud
may have been committed.

| What information to provide |

When completing the online Fraud Allegation Form, you will be asked to provide all of the information you have
pertaining to the suspected fraud, including the following:
- Name and address of person you are reporting;
- Details about the improper or fraudulent activity
you suspect;
- Other general information pertaining to the suspected fraudulent activity.
We designed the online form to allow you to easily provide specific
information. Those specifics allow us to efficiently conduct a thorough, fair and objective
investigation of the fraud subject. While it is unlikely that you will have all the data requested,
we ask you to provide all the information you have pertaining to your fraud allegation.
