Any party to the claim may file an appeal. The party must
file the appeal in writing, and it must be received within the appeal period.
BWC will not accept a verbal appeal.
You may use the Notice of Appeal (IC-12) to file an
appeal. BWC accepts any appeal as long as it contains the
following information:
Name of the injured worker and employer;
Claim number;
Date of the order being appealed;
Reason for appeal.
You can get an IC-12 at any BWC
customer service office or IC hearing office. Sign and date the appeal, then
mail, fax, or deliver it to the local BWC customer service office or IC office.
