How to report an injury

Prior to the Health Partnership Program (HPP), injured workers, providers and employers filed claims with the First Report of Injury (FROI) application. Under HPP, it’s just as simple — call the employer’s managed care organization (MCO) to report an injury.

If you do not know which MCO is assigned to the employer, ask the injured worker or employer. You can also use the Employer/MCO look-up on this Web site.

When you report the injury to the MCO, the MCO representative either will take the necessary information over the telephone or ask you to fax a completed FROI.

You also can file the FROI online and immediately receive a claim number. To order FROI applications, call 1-800-OHIOBWC, and follow the options or download the application from this Web site.
