Provider enrollment and certification
To be eligible for reimbursement, (except for state-fund claims with dates of injury prior to Oct. 20, 1993,
emergencies, initial visits or as otherwise defined in the Ohio Administrative Code (OAC) 4123-6-12) medical
providers must be BWC-certified. The first step in becoming BWC-certified
is completing and submitting the Application for Provider Enrollment (MEDCO-13)
along with all required documentation. BWC will review the information to ensure that you meet the
minimum certification criteria.
Providers must meet all licensing, certification and accreditation requirements necessary
to provide services. Other minimum credentials are based on provider type. If you
meet all the credentialing criteria and sign the provider agreement,
BWC will certify you to participate in the Health Partnership Program (HPP).
