How to report an injury

You can file the First Report of Injury (FROI) online, call the employer’s managed care organization (MCO) to report an injury, or complete and fax the FROI to the MCO’s toll-free number found in the MCO Directory.

If you file the FROI online, you'll immediately receive a claim number. To order FROIs call 1-800-OHIOBWC and follow the prompts, or download the application from this Web site.
Complete the FROI online.

If you do not know which MCO is assigned to the employer, ask the injured worker or employer. You can also use the Employer/MCO look-up on this Web site.

When you report the injury to the MCO, the MCO representative either will take the necessary information over the telephone or ask you to fax a completed FROI.
