Provider enrollment and certification
You must be a BWC-certified provider to receive reimbursement for your services, except
for the following circumstances:
- Ongoing treatment of an injured worker with date of injury before Oct. 20, 1993,
in which treatment began prior to the Health Partnership Program. The injured worker
may continue treatment with a non-certified physician. However, an MCO will manage
the care;
- Emergency treatment;
- Initial treatment - Non-BWC certified providers must have a BWC provider number
to receive reimbursement for services. If the injured worker changes physicians,
he or she must select a BWC-certified provider or the injured worker will be
responsible for payment;
- Specific provider types listed on the Application for Provider Enrollment
Non-Certification. (MEDCO-13A)
To become BWC-certified you must complete the
Application for Provider Enrollment (MEDCO-13) along with all required documentation.
BWC will review the information to ensure that you meet the
minimum certification criteria. Providers must meet all licensing, certification and
accreditation requirements necessary to provide services. Other minimum credentials
are based on provider type. If you meet all the credentialing criteria and sign the
provider agreement,
BWC will certify you.
All BWC-certified providers are listed on this Web site. Visit the
BWC-certified provider look-up in the Medical Providers look-up section.

Pharmacy provider enrollment is a service provided by BWC’s Pharmacy Benefits Manager
(PBM). Pharmacy providers should contact the
PBM to enroll.
