Minimum criteria to become a Disability Evaluators' Panel (DEP) physician

  • BWC-certified provider in good standing with the Health Partnership Program (HPP)
  • No suspension or probation from professional practice by any agency of competent jurisdiction within the past five years
  • Board certified as recognized by the following:
    • Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) who is board certified by a specialty board recognized by the American Board of Medical Specialties or the American Osteopathic Association;
    • Doctor of Chiropractic (D.C.) who has obtained diplomate status in orthopedics, neurology, internal disorders, sports medicine, rehabilitation or occupational health as recognized by the American Chiropractic Association;
    • Psychologist (Ph.D.) who has three years experience in health psychology or behavioral medicine, or one year post doctoral training and two years clinical experience in health psychology or behavioral medicine;
    • Doctor of dental surgery (D.D.S.) who is board certified in maxillofacial or oral surgery;
    • Podiatrist (D.P.M.) who has diplomate status with the American Board of Podiatric Surgery.
  • Maintains at own cost and expense a valid current policy or policies of professional liability insurance with minimum coverage of $1 million to $3 million and comprehensive general liability insurance with minimum coverage of $500,000
  • Maintains workers' compensation coverage in Ohio for Ohio location
